All Levels, Boss Levels (including Hard Mode for all Bosses), and Side Levels are S-Ranked with all Skills collected, all collectables found, all Chaos Emeralds Collected and all Trophy Codes Entered in the Secret Museum, (hold the Back button on the Xbox 360 Controller while in the Gallery Room to access the secret room). The original Sonic the Hedgehog Genesis game is unlocked as well.
