That seems very lazily made, first off because just plainly increasing enemy health isn't at all a good method of making a higher difficulty, and second, no items seems incredibly odd because iirc in final fantasy games your only options are really 1. hit things and 2. use items, haven't played...- Mama Looigi

I would never enjoy playing a game just for the sake of it being a challenge, I genuinely love how Elden Ring's combat feels and the game is so open and you can get so many great items early that it pretty much eliminates the whole "Unfair" feeling- Mama Looigi
