As far as differences go, I see just three:
-he now wears gloves
-belly is bleached
-his eyes are wider
-he now feels happier
(okay, potentially one or two more...but those could be from getting hit in the face)

Doesn't really look like much surgery to me. The latter two seem like Robotnik just put Sonic on drugs.
